DMAIC is the structured problem-solving roadmap used in Six Sigma projects to systematically investigate and resolve process quality problems. The five phases are sequential and build upon each other: Define establishes the project scope, business case, customer requirements (CTQs—Critical to Quality), and project charter with executive sponsorship. Measure documents the current process, confirms measurement system validity (through gauge R&R studies), and establishes baseline performance metrics. Analyze uses statistical tools (Pareto analysis, fishbone diagrams, regression, hypothesis testing) to identify the vital few root causes that are statistically confirmed to drive the defect or performance gap. Improve designs and pilots solutions that eliminate root causes. Control implements ongoing monitoring systems (control charts, mistake-proofing, updated SOPs) that sustain the improvement. The Measure phase is frequently underinvested—teams eager to fix problems skip rigorous baseline measurement and jump to solutions. This creates two problems: no clear understanding of how bad the problem is before intervention (making it impossible to quantify improvement), and solutions applied to assumed rather than confirmed root causes. Gauge R&R studies (measuring measurement system repeatability and reproducibility) are particularly important in manufacturing environments where unreliable measurement systems can masquerade as process variation, leading teams to solve measurement problems rather than process problems. The Control phase is the most commonly failed phase in practice. Teams successfully implement improvements in a pilot or short-term validation period, but improvements erode when the project team moves on and day-to-day management reverts to pre-improvement behavior. Sustainable control requires four elements: updated process documentation (SOPs), implemented mistake-proofing (poka-yoke—physical or digital mechanisms that make errors impossible or immediately visible), statistical process control charts with defined response plans for out-of-control signals, and management accountability reviews that monitor the controlled process metrics in ongoing operations.
